THE SCULPT SOCIAL — PARTICIPANT LIABILITY WAIVER

By checking the box below, I acknowledge and agree that participation in Pilates classes, strength mat sculpts, events, and exercise activities led by The Sculpt Social (Rae Musitano and its instructors and representatives) involves physical exertion and inherent risks of injury.

I represent that I am in good physical condition and have no medical impairment that would prevent my safe participation. I assume all risks associated with participation, including but not limited to temperature changes, uneven surfaces, wind, sun exposure, and contact with other participants.

I hereby release, waive, and forever discharge The Sculpt Social and its instructors from any and all liability, claims, demands, or causes of action arising out of my participation in these classes and events.

PHOTO & VIDEO RELEASE

I understand that photographs and video recordings may be taken during classes and events hosted by The Sculpt Social for promotional, marketing, educational, and social media purposes.

By participating, I grant The Sculpt Social permission to use, reproduce, edit, and publish any photographs or video footage in which I may appear across its website, social media platforms, advertisements, and other marketing materials without compensation.

If I do not wish to appear in photos or videos, I understand it is my responsibility to notify an instructor before the class begins. While The Sculpt Social will make reasonable efforts to accommodate this request, it cannot guarantee that I will not appear incidentally in group footage.
